Position Title:    Clinical Nurse - Post Acute Rehab Aged Care (Palliative Care) - Thursday Island

Location: Torres and Cape Hospital and Health Service

Remuneration:  

Base comprising salary between $96,909 - $103,768 p.a,  employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (Nurse Grade 6).

Duties:

Your role will assist in the improvement of the health of people living in the Torres Strait Islands to equal that of mainland Australians; provide the coordination and delivery of quality cost effective Primary Health Care and Palliative services.

This position is placed within the Division of Primary Health Care, Thursday Island within the Post-Acute Rehab and Aged Care Program.

To be successful in this role you will have:

• Demonstrate knowledge of Aboriginal and Torres Strait Islander culture, including application of the Torres Model of Care.
• Demonstrate ability to provide community support and awareness surrounding care of the Palliative client.
• Demonstrate culturally responsive appropriate palliative care at an advanced level
• Demonstrates quality improvement processes and ability to apply validated research to clinical practice
